    The Visiting Health Services department is seeking an RN to join their team.


    This is a part time position, scheduled 20 hours per week and will be eligible for a comprehensive benefits package OR a 13% pay differential in lieu of benefits. 


    Provides age-appropriate skilled nursing care to patients in their place of residence in accordance with agency policy and procedure and state and federal regulations. Teaches the patient/family disease process and self-care needs. Assess physiological state to determine an appropriate level of safety and comfort. Provides services requiring nursing skills in accordance with the plan of care. Develops and coordinates the plan of care and provides case management. Performs duties consistent with the nursing practice act that includes the standards of competent performance including Oregon and Washing State Home Health Nursing Regulations.

    Candidates must possess positive interpersonal skills in order to effectively communicate with patients, families, physicians and other healthcare staff. A passion for Planetree patient-centered care is essential for this position. An essential function is working as a team member with a variety of different disciplines to provide high quality home based care. . Must be able to drive long distances, drive in extreme weather, and have access to high speed internet, the ability to use/learn EMR and access to a cell phone.


    Minimum Education, Training & Experience (includes licenses or certifications):



    • Associates of Science in Nursing from accredited school
    • Current Oregon and Washington RN License or ability to obtain
    • Valid Drivers License and Auto Liability


    • One year of acute care and/or home care experience preferred or the equivalent.
    • Current Level C CPR Certification
    • Experience with wound care, home IV, medication management, cardiac management and diabetes management preferred.
    • Experience in Home Health, long term care or Public Health with-in the last year is required, as well as a basic understanding of Medicare and Joint Commission rules


    • Previous experience in hospice, home health, geriatrics or general medical/surgical nursing.
    • Must be able to work independently Must have an in-depth and up-to-date knowledge of nursing theory and practice and the legal and ethical issues associated with them
    • Should be neat in work and appearance; be tactful, courteous, and ethical.
    • Requires critical thinking skills, decisive judgment and the ability to work with some supervision. Must be able to work in a stressful environment and take appropriate action.
    • Knowledge of Medicare regulations/requirements
    • Working knowledge of palliative care for the terminally ill patient
    • Knowledgeable of the care for the chronically ill and frail elderly
    • Excellent time management, problem solving and customer service skills
    • Excellent written and verbal communication skills
    • Able to work autonomously and manage the case load
    • Must have capability to access and use high speed Internet
    • Operational knowledge of computerized clinical documentation including the use of the laptop/notebook computers.
