  • Overview

    Mid-Columbia Medical Center is currently seeking a Care Manager (RN) to join our Performance and Quality Systems team.

     

    This is a full time position and is eligible for a comprehensive benefits and time off package.

     

     

    Responsibilities

    Under minimal supervision, this position manages patients through health care systems, coordinating the delivery of quality health care services across a continuum of care.  Balances individually identified patient and family needs with the efficient and cost effective use of resources, emphasizing patient and customer (i.e. physician, employee, payor, vendor, community) satisfaction.  Continually monitors, evaluates and assists modifying the treatment plan to achieve optimal patient outcomes.  Monitors and updates care paths; may provide for utilization review, promoting continuous quality improvement.  This position is considered the subject matter expert for physicians. 

    Qualifications

    Education: 

    • Registered Nurse (RN) licensed in the State of Oregon; BSN preferred OR MSW; LCSW
    • BLS

    Experience:     

    • Three (3) years of clinical hospital experience, preferably in the local area.
    • Ability to understand illness, its effects on patients, family and the community.
    • Possess an awareness of the social ramifications of illness including an ability to implement plans to improve a patient’s situation.

    Knowledge/Skills/Abilities: 

    • Proficiency with the use of Electronic Medical Record Software (Epic System) and Microsoft Office. 
    • Ability to work efficiently with minimal supervision, exercising independent judgment within stated guidelines.
